Physical Therapist - Home CareCare New England Corporate HR
The Physical Therapist is accountable for effectively managing a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement.
Education: Equivalent education required for RI licensure required, Bachelor’s Degree (min) in relevant discipline strongly preferred.
Experience: A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or Home health care environment setting preferred.
